Thank you for registering on the MED-EL. ("MED-EL" or "we")
HearPeers web site. Your submission, access to and use of this site is
conditional upon your acceptance, without modification, of the terms and conditions
of the HearPeers Website. By accepting these, you acknowledge that you
have read and understood, accept and agree to be bound by, the following terms

as part of the HearPeers program and web site (each a "Submission")
you are granting MED-EL, its affiliated companies and any sublicenses a revocable,
non-exclusive, royalty fee, world-wide license and permission to use your Submission
in connection with MED-EL's business including, without limitation, the rights
to: copy, distribute, transmit, publicly display, reproduce, edit, translate and
reformat your Submission (both on MED-EL's web site or in paper copy). You acknowledge
that you are not entitled to any fee, commission, payment, or other remuneration
as a direct or indirect result of your submission. 4.
Information you provide must not infringe any copyright or be offensive.

HearPeer Program AUTHORIZATION
FOR USE OR DISCLOSURE OF PROTECTED HEALTH INFORMATION 1.
I authorize MED-EL Corporation to use or disclose my protected health information
as described in this authorization. The specific Information that I authorize
MED-EL Corporation to use or disclose is my e-mail or other contact information
listed below along with the fact that I am a recipient of a Cochlear Implant,
and all pertinent information concerning the type, model number, and other data
related to the characteristics of my Cochlear Implant. 2.
MED-EL is authorized to release my information to each person who visits the MED-EL
web site and indicates an interest in obtaining information about Cochlear Implants.
I understand that MED-EL will direct those visitors to its web site who want to
contact a current recipient to get information about Cochlear Implants, without
further investigation or qualification by MED-EL to me and or other "HearPeers." The website visitors will be directed through a series of questions
that solicit information about the general nature of their hearing problems; and
depending upon the information such a person provides, my e-mail address or other
contact information may be furnished to any such person.
3.
I am giving this authorization so that MED-EL can identify me as an informational
resource for other persons who are considering the possibility of a Cochlear Implant,
whether for themselves or for someone else. I understand that as a result of MED-EL's
use and disclosure of my information to these other persons, it is possible that
the individuals involved may decide to receive a MED-EL Cochlear Implant and that
as a result, MED-EL may receive income from the sale of the Cochlear Implant and
any other related goods or services. I also understand that each of the points
listed on the reverse side of this form apply to this Authorization.
I
understand each of the following seven points apply to this Authorization:
1.
I may refuse to sign this authorization and MED-EL will not condition treatment
based upon my providing a signature on this authorization. 2.
Any information used or disclosed because I have signed this authorization may
no longer be protected by privacy laws and may be subject to re-disclosure by
the person or organization receiving it. 3.
I have a right to revoke this authorization at any time by doing so in writing
and presenting my written revocation to the Medical Records Department of MED-EL
Corporation. 4. Any request to revoke my authorization
will not apply to actions that MED-EL Corporation has taken action in reliance
upon my authorization. 5. Any request to revoke
this authorization will not apply to my insurance company when the law provides
my insurer with the right to contest a claim under my policy. 6.
I may inspect or copy any information to be used or disclosed based upon this
